How many months ago did(name)take the last dose?
Interviewer instructions
If the answer to VA1 was 'Yes', enter the number of months ago the last dose was given to
the child. As you have done before, use a leading zero if necessary. If the child received
the supplement during the one month period preceding the interview, enter '00'. Circle '99' if
the mother/primary caretaker does not know when the child took the last dose.
