If the child was taken to a health facility because she/he was sick, and the supplement was obtained during this visit, circle '2'. If Vitamin A supplement was given during a National Immunization Campaign, circle '3'. If Vitamin A supplement was given at home, circle '4'. If the supplement was obtained from a source not listed, write the description next to 'Other' and circle '6'.
